My son had a child at 16, the mothers parents had told him to sign some papers about insurance?

The papers wound up being papers for guardianship. I told the girls mother prior to the papers being signed he was not to sign anything. The papers he was shown to sign and the papers that were filed in court are two different papers. He basically signed his rights over without knowing. Are these papers legal and binding due to his age?
