Helpppppp me please serious problem?

my mom and i do not live together, actually in different states, only 20 mins away though i live with my boyfriend. i am almost 16 and pregnant and i need to go to the doctors alot and im currently needing to go to the hospital bc i know i have a bladder infection and im in so much pain. my mom refuses to take me sue to her laziness and just not caring, she gets money for me for food stamps and spend none of it on me and all on herself. i need to get wic and food stamps for myself for the baby but i dont want to get it through her because she doesn't spend it on me now and i know she wont then, i wanna be able to get to the doctors on my own in my own state by myself. i havent lived with her since i was 10 and that's because her boyfriend kicked me out of their house. she tells everyone its because i didn't wanna live there and go to them schools that's why i moved to my grandfathers in a different state, i guess to make herself look better. she never takes care of me and i hate not having the option to take care of myself, i want whats best for me and my baby. i am just curious if i went to the welfare building by my home, would they be able to help me, i dont wanna loose my insurance that i already have through her than not be able to go to the Dr's for me or my baby. my boyfriends mother had legal guardianship of me. i am still in school and i just need help, im not eatting healthy due to my mom not sharing the food stamps and im in alot of pain and need medical attention and she doesn't care to take me to the hospital and i need my guardian because of my age everywhere i go. what can i do and how can i do it ?
