What is the safest way to legally change your name ?

I cannot find a free form for petition of name change and an order file form, for my county. Has anyone ever done this themselves and was it easy? Who do I notify once my name is changed ? Would it temporarily mess up my medical card or health insurance. I am my brother's guardian, would that mess up his social security check or my guardianship ? Thanks, for any help ( :
